How much does a late Form 5472 filing cost?

$947 per tax year if the LLC was dormant that year (no transaction between you and the company), or $1046 per tax year if it was active (a transaction occurred — a contribution, distribution, or a fee you paid personally on the company's behalf). Both are all-inclusive: the Form 5472 and pro forma Form 1120, the reasonable cause statement, transaction review, IRS filing, and written confirmation.

What about multiple years?

Each year is priced on its own — some years may be dormant, others active. All years are quoted together before you pay.

Why not one flat number?

An active year takes more work: reviewing the actual transaction and making sure the reasonable cause statement addresses it correctly. A dormant year — nothing moved — is simpler to prepare, so it costs less.

Why not $299 like a standard filing?

A standard on-time filing is a different job. A late filing needs a reasonable cause statement attached to the return, which is the difference between a $25,000 penalty and no penalty.
